ID Card Application Form

I.

To get a Tribal ID: (1) please fill in the information. (2) Sign in the Signature Block, and (3) send in this form, along with a passport photo and a $11.00 processing fee ($10.00 per card + $1.00 per card postage & processing). A separate form, photo and fee need to be submitted for each person. Please do not wear a cap or hat in photo and do not paper clip photo to paper.

Signature Block:   Please sign your name inside the Signature Block using a black Sharpie Fine Point Permanent Marker or similar pen. Make sure your signature fills up the entire block. Parents may sign for minor children. Do not sign with an Ultra Fine Sharpie.

FAQs About ID Card Application

(1) You must be a member with a tribal number before you can order a card.

(2) You do not need an ID card in order to be a member of the Tribe.

(3) You MUST submit a color Passport Photo NOT one taken with your camera.

(4) Make the $11 check or money order out to "Lipan Apache Tribe of Texas" NOT Robert Soto.

(5) All applications are mailed to Vice-Chairman Robert Soto at his home address. (Look on Application)

(6) Your signature MUST fill the entire signature box! (See example on the application)

(7) DO NOT use an ultra-thin/thin felt tip pen for your signature...your signature will then be too thin to be copied by the ID machine.

(8) Be patient! Cards can take a long to make. If your check has been cashed (after 5/2010) and you have not been notified of problems, then your card will be made in the order the appliation was received. Email Chairman Soto at robtsoto49@lipanapache.org with questions.
